Roofing company fined after worker dies on the job

Toronto-based roofing company Crawford Roofing Corporation has received a $150,000 fine for violating the Ontario Occupational Health and Safety Act after a worker fell to their death through an unprotected skylight opening.

Toronto-based roofing company Crawford Roofing Corporation has received a $150,000 fine for violating the Ontario Occupational Health and Safety Act after a worker fell to their death through an unprotected skylight opening.

The incident took place on May 14, 2010 when workers had just completed applying a vapour barrier over a skylight opening when another worker began to move forward and into the opening.

Despite the effort made by the other worker to make a warning, the individual stepped onto the paper and fell through the skylight opening, falling 20 feet to the concrete floor below.

The worker died from head trauma from the fall.

The company pleaded guilty to failing, as a constructor, to ensure that workers working around an opening were properly protected and that appropriate warning signs were posted.
