The Ontario Ministry of Labour is undertaking an investigation into the death of a worker who was crushed at the Herb Gray Parkway construction site in Windsor, Ont.

“The constructor, Parkway Infrastructure Contractors, reported that the worker was found with critical injuries trapped between two objects,” said William Lin, spokesperson with the Ontario Ministry of Labour.

“The worker was removing a steel concrete form at the time, when the form work fell on the worker, resulting in fatal injuries. And, the worker succumbed to those injuries.”

The incident occurred on June 17 around 4 p.m. on the construction site of the Herb Gray Parkway project at tunnel 7 southwest, near Todd Lane and Huron Church Road.

The man is employed by PCR contractors. As a result, emergency medical services were dispatched and attended the scene. The coroner also attended the scene.

“The Windsor Essex Mobility Group and Parkway Infrastructure Constructors are saddened to confirm a fatality of an employee of one of the subcontractors has occurred on the Rt. Hon. Herb Gray Parkway,” said the two companies in a statement.

“Our thoughts and prayers are with the families at this time. We are participating fully in ongoing investigations and will provide further information upon its completion.”

The Ministry of Labour dispatched an inspector to the scene on June 17, who issued four orders to PCR Contractors, including a stop work order.

One order requires the employer to ensure that every part of the project including temporary structures are adequately braced to prevent collapse.

Another order requires that workers are provided with instruction, training and supervision with respect to safe work procedures, for the installation and removal of pre-manufactured steel concrete forms.

The final order requires the contractor to provide documentation, including a list of workers, drawings, design specifications, safe work procedures and technical documents related to work at tunnel seven.

Two orders were issued to Parkway Infrastructure Constructors. The first order requires the constructor ensure that all contractors and workers on the site work in compliance with the Occupational Health and Safety Act and its regulations. The second order requires the constructor to provide proof of worker safety orientation training, safety meetings, constructors safe work procedures, health and safety policies and emergency procedures.

The Rt. Hon. Herb Gray Parkway is a $1.4-billion highway infrastructure project, which is currently under construction. It is designed to improve traffic flow in Canada’s busiest trade corridor.

The project is being constructed through an Alternative Financing and Procurement model under a design, build, finance and maintain contract with the Windsor Essex Mobility Group and their joint venture Parkway Infrastructure Constructors. The project spans an 11-kilometre corridor and has three components:

— a six-lane below-grade freeway which is an extension of Highway 401

— a four-lane at-grade service road network which is an extension of Highway 3

— 300 acres of green space which will include 20 kilometres of recreational trails, ecological restoration sites and thousands of new native trees and vegetation.

Upon completion, the parkway will move people and goods to and from a proposed new Canadian inspection plaza and international bridge.

Construction started in 2011 and it is estimated that the project is generating about 12,000 jobs. It is anticipated that the parkway will be open to traffic in late 2014.
